APPLICATION FOR GRADUATION

INSTRUCTIONS:

| Graduation means receiving the diploma and having the transcript
annotated with degree & date. |
- If you will complete all requirements for your degree by the end of Spring term in May, you will be eligible to graduate at that time. You may choose to participate in the May 11, 2013 ceremony or not.
Please mark your application accordingly. Caps and gowns must be ordered at the end of February,
and we order only for those who indicate they will participate.
- You must complete the application process to ensure a diploma
is ordered for you. The application form initiates the process; no action
will be taken until it has been submitted.
- You must apply for graduation and pay the graduation fee.
- The deadline for Spring 2013 applications is February 14, 2013. You MUST meet the deadline.
- The graduation fee
is $100.The fee will automatically be charged to your student account. The account must be paid in full before the diploma will be released.
- For those attending the ceremony, and will have completed the degree requirements, diplomas will be given out at the ceremony. All others will be sent via certified mail with signature required to ensure delivery by mid-July. You may arrange to pick yours up in person if you prefer.
- NOTE: Any holds will prevent our releasing your diploma and transcript.
